A printable weekly tracking log for mood, sleep, anxiety, energy, and medication — designed to make your medication visits more productive and help you identify patterns.
Tracking your mood and symptoms between appointments gives your provider concrete data — not just how you are feeling today, but how you have been feeling over time. This tracker is designed to take less than 2 minutes per day.
How to Use This Tracker
- •Fill it out once daily — morning or evening, whichever you will actually do
- •Rate each item on a 1–10 scale (1 = very low/severe; 10 = excellent/none)
- •Add brief notes in the 'Notes' column — a few words is enough
- •Bring the completed tracker to your appointment
